Responsibilities:

  • Approach colleagues and all job-related activities from a perspective that recognizes and honors diversity, equity, and inclusion.
  • Provides comprehensive psychosocial assessment of all clients to inform treatment.
  • Assists in the development of short-term and long-term behavioral health goals for assigned caseload. Makes recommendations to the Treatment Team regarding service needs, treatment issues, and progress of the individuals.
  • Attends Individual Service Planning conferences of assigned caseload. Prepares a summary of treatment and documents treatment as it relates to clinical/behavioral health goals.
  • Provides ongoing case clinical case management for individuals within the adult services department. Provides comprehensive data for psychiatric appointments and attends appointments for additional advocacy as needed.
  • Provides or coordinates individual 1:1 counseling service as determined on an individual case basis.
  • Conducts client-focused group skill development curriculum for individuals as needed.
  • Provides direct training and support to direct support staff members on behavior plan(s), data collection, and therapeutic interventions as needed.
  • Regularly informs families and referral agencies of individual's progress and treatment needs as they relate to clinical/behavioral health needs.
  • Develop and implement training modules for individuals served in the program.
  • Participates in program meetings, which include Medication Reviews, ISP reviews, Service Plan Reviews, and Program Team Meetings. Communicate information to members regarding family issues, treatment needs, and updates on treatment goals.
  • Maintains interdisciplinary contact with psychiatry, nursing, residential program, and vocational or day placement.
  • Documents clearly and consistently all case-related issues and treatment summaries and maintains an accurate recording of case events in the individual's record.
  • Attends trainings, lectures, and courses as indicated, as part of professional development.
  • Participates in performance quality improvement efforts that include but are not limited to timely and accurate completion of identified workflows and documentation measures outlined in the agency PQI plan and divisional goal plans.
  • Participate in an agency-wide Clinical Leadership Team representing the clinical interests of Adult Services
  • Other reasonably related responsibilities as assigned by the VP of Adult Services and/or VP of Clinical Services.

Requirements:

  • Minimum of one-year clinical experience.
  • Experience working with individuals with intellectual and/or developmental disabilities.
  • Master's Degree in Counseling, Mental Health Counseling, Social Work, ABA
  • BCBA, LMHC, LCSW preferred

Monday - Friday 9a-5p

Pay Range: $62,000 - $68,000 per year
