Overview

The Population Health Management Consultant will be a part of the Business Analysis Department and assist customers with implementing the various Population Health Management (PHM) solutions offered by eClinicalWorks. We are expanding our PHM Consulting Team to include a Behavioral Health expert to help implement our solutions for outpatient behavioral health management and 24-hour care setting. The PHM Consultant will also work with the customer to establish a project plan that aligns with their strategic goals, help customize the product, process, and workflows as well as train end-users to successfully use the eClinicalWorks PHM solutions for their requirements. Each consultant will have their own list of projects and will be responsible for making sure that their clients receive any assistance required to successfully participate/satisfy the requirements of the selected Population Health Management initiatives. Occasional travel to customer sites will be required.

Responsibilities

  • Assist in the implementation and adoption of Behavioral Health solutions with new and existing clients.
  • Assist in the implementation of other Population Health Management solutions that a customer may opt for.
  • Assist customer with implementing community based mental health concepts including group therapies.
  • Understand and research any customer-specific requirements related to Residential Behavioral Health, Care Management in a 24-hour care setting and other similar areas.
  • Understand and research any reporting requirements that may be needed and working with internal teams accordingly.
  • Work with internal stakeholders in evaluating additional customer & industry requirements (Design and Development).
  • Identify gaps in existing processes/workflow based on program requirements and help identify key system workflows and best practices.
  • Develop strategic goals, objectives, and timelines for all Population health initiatives implemented by a customer.
  • Create customized documents for system build and workflows.
  • Educate customer on federal and state-specific reporting requirements.
  • Develop training curriculum and work with eCW trainers to execute successful training sessions.
  • Assist with the application/recognition process and other actions needed to achieve compliance with the programs identified.
  • Assist with post-submission audit support when necessary.
  • Educate and assist customer team(s) on improving product adoption/usage regularly after go-live.

Requirements

  • Licensed Marriage and Family Therapist, Licensed Psychologist, Licensed Clinical Mental Health Counselor, or Licensed Clinical Social Worker. 5 years' experience working in Behavioral Health outpatient and/or 24-hour care setting.
  • 2 years of experience with the implementation of an Electronic Health Record product in an ambulatory and/or acute care setting, either as a member of the clinical project team or as a clinical super-user.
  • Knowledge of group behavior and dynamics, societal trends and influences, human migrations, ethnicity, and cultures and their history and origins.
  • Familiarity with PHM Tracking and reporting tools.

Preferred Requirements

  • Knowledge of managed care experience preferred.
  • Client-focused and willing to go beyond regular responsibilities to ensure a successful implementation.
  • Show initiative and work on projects with minimal supervision.
  • Understand customer limitations and suggest alternatives/workarounds.
  • Knowledge on insurance reimbursement models is a plus.
  • Familiarity with Population Health Management programs / initiatives such as ACO, PCMH, CCM, TCM, RPM, HEDIS, HCC, etc., is a plus.
  • Excellent analytical, written, and oral communication skills.
  • Ability to articulate facts and ideas clearly and concisely.
  • Excellent customer service and interpersonal skills.
  • Working knowledge of Microsoft office and related products
  • Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental disfunctions, and for career counseling and guidance.
