Job Summary: The Chelsea Community Center Permanent Supported Housing Program provides 24/7/365 services to adult men who come into the program following extensive periods of homelessness. These individuals require very low threshold supports given their experience of homelessness, substance use and other behavioral health and complex medical conditions. The program provides these low threshold housing services in a manner that addresses the specific needs of the individuals through engagement in care and assessment of needs with the goal of sustaining safe and affordable permanent housing. The program is predicated on trauma informed care, harm reduction, and housing first principles. Bay Cove and the program team are committed to offer services in a manner that fully supports the dignity and respect for each person and works within a framework that provides for individualized supports aligned with each person's needs and preferences. The Director supervises all team members and role models work with all tenants, partners, staff, and other community stakeholders in an affirmative and inclusive manner, fostering safety, successful tenancies, and overall wellness for resident.
Supervisory Responsibilities:
Supervision of 3 FTE Case Managers, Safety Specialist Supervisor
Schedule & Employment Status:
The CCC Program Director is a full time (40+ hours) position. The Director is expected to work a noon - 8 pm schedule on Sunday - Thursday, in order to fully support residents, staff, and the program mission. Like all team members, the Director is expected to share in holiday coverage and on-call. This is an exempt position.
Essential Functions of Position:
*Supervises all permanent and per diem personnel, ensuring that staff receive extensive onboarding in their roles and responsibilities upon hiring, and that staff receive individual supervision as outlined in Addictions and Homeless Services guidelines, ongoing feedback regarding their performance and performance appraisals that maximize their job success and excellent services to residents.
*Works closely with residents, the team, and community and collateral providers to ensure safe and appropriate services and access to care for all CCC tenants.
*Provides on call to the program to ensure for sufficient support, direction, problem solving and to address urgent or emergency needs. While the program is in expansion, on call will likely be shared with the
*Leads the program management team to plan, outline, organize, monitor and report on goals, objectives and outcomes for the program.
*Assures that the program adheres to the service delivery contractual agreements and Bay Cove Human Services Inc. policies and procedures.
*Oversees documentation systems required of the program including documentation in the clients' charts (paper and electronic) inclusive of Comp. Assessments, Service Plan, and contacts/interventions made of behalf of each person, needed health and medical information. Ensures that critical incidents are reported to Bay Cove and the Bureau of Substance Addiction within as required.
*Models respectful, affirmative, and inclusive supports and interventions informed by evidence-based and emerging best practices (trauma informed care, harm reduction, engagement strategies for homeless adults, and housing first principles in the delivery of low-threshold services to all residents.
Responsible for ensuring the program's adherence to relevant federal and state and municipal policies.
*Works collaboratively with Supervisor, Addiction & Homeless Services leadership, CCC Property Owner, MA Housing & Shelter Alliance staff , and Bay Cove Agency leadership to fulfill program mission
*Supports agency Diversity, Equity, and Inclusion initiatives and practices to strengthen services for persons served and workforce.
*Performs varied other duties as requested or by supervisor.
Job Requirements:
*Master's degree preferred. Bachelor's degree and CADAC certification can be substituted for Master's. Strong writing, interpersonal communication, and computer skills required (Word, email, Excel, electronic health record, HMIS)
*Minimum 4 years of experience working with adults with behavioral health challenges and/or homelessness.
*Demonstrated experience working within low threshold housing for formerly homeless adults and being able to effectively de-escalate persons who are overwhelmed and/or under the influence of substances.
*Minimally one year of supervisory and management experience
*Able to lift and carry up to 30 pounds.
*Able to walk up and down three flights of stairs several times daily to assess potential client needs and safety issues.
*Able to visually observe and respond to clients in medical and/or behavioral health distress including performing CPR, administering Narcan or other First Aid, and using an AED properly
